New Client Forms
Here you will find the list of forms needed for an occupational therapy evaluation. Download all forms and complete them fully, signing where needed. Bring these items with you to your appointment along with your doctor’s prescription for your child to receive services. If you have not obtained a referral from your child’s physician yet, please contact your doctor’s office and ask for a doctor’s referral for OT services. The prescription should say “Occupational Therapy evaluation and treatment” along with any diagnosis your child has.
